cartrige stop

I know this probably has been asked before but I cant find any info on it, so here goes with the question. This has been eating me up, sometimes when I lever my 357 fast I eject a spent round and a loaded one at the same time. Ive read that what might cause this is the cartrige stop not holding the round in the magazine and letting out two at the same time. When I read this the author did not say how to fix it.
any help sure will be appreciated.

And by the Way I call Steve Young and his wife says on the answering machine that he has surgery but is doing fine.

Well, that's kind of easy enough, is there a round still on the lifter when the empty and the loaded round eject?

Rossi "Puma" 92, 357/38 a new one, rifle is about 3 months old
