A dear friend had a stroke this evening.
One of the nicest guys I've ever known, and a brilliant physician who is dedicated to helping patients others often shun or ignore.

His name is James. He is the kind of crazy doc like me who just wants to help people. Hasn't gotten paid in over six months due to our rotten 'insurance' system, but he keeps doing the right thing, hoping the 'system' will get fixed.

Jim passed away last night. I sure will miss the guy. He was not only a great doctor, but just an all-around nice person. We both even liked the same weird music (jazz, classical, folk) and would drive out staff nuts listening to it, instead of the top-40 stuff they always put on...
I'd been riding herd on about 3,000 patients lately, working 85-90 hour weeks for no pay*, and he was trying to build his practice, so combining forces would have helped us both. Now instead of his helping me with my workload, I'm going to have to help his patients the best I can (both our practices involve complex patient care most other primary care physicians simply won't do). Our NP had already emailed me on Sunday to tell me the coming Friday would be their last day because they took a better paying job, so I was doing double-duty. Now it will be triple duty. So I will also miss Jim in that way (how dare he die on me, and not stick around to help). I guess God had plans for both of us we had little say in. I know his family will do ok, but his patients are in a bad situation, because nobody else wants to see them, and I can't exactly add more onto my schedule when I work a 16 hour day.
(*when you have that many patients, in primary care, there is a given amount of unreimbursed work like phone calls, Rx refill requests, prior authorizations, etc., that has to be funded by enough office visits, and you really need a provider for every 1,000 patients or so to provide all the office visits they need. I'm the only provider, so I'm the only one able to do 'billable services' to fund all the other free stuff we have to do. Unfortunately, new grads and NP's usually want you to pay/guarantee their first year's salary, even though they won't bring in any revenue for the first six months due to insurance delays. I've borrowed several hundred thousand dollars over the years to 'help the new one get started', and never recouped it, because once you salary/guarantee someone like that, they get lazy and never really work that hard.)
Anyway, Jim was able to do what he enjoyed right up until the last day, so at least that much is good.

AJ, I understand about the non-billable services. You probably remember calling around to find a medicine for a patient when they needed it immediately and you didn't have it on hand. I have spent a lot of time doing things like that so that the patient could be helped, and rarely received as much as a thank you. I work for a corporation because most independent pharmacies just can't make it with paying higher prices for the medicines and getting paid lower reimbursement rates by insurance companies than the chains. If the playing field were level, then the service and attitude of the provider would be able to drive the business. Insurance companies and laws which let them fail to live up to contracts sure hurt providers and patients.
