Does anyone make scout scope mount for an 1886 in 45-90?
It's a 2024 so I have no qualms about having the barrel drilled.

You may be limited to a picatinny rail.
Skinner Sights has them for other leverguns.
But even if you mounted a rail that fit the contour of your barrel and it sloped too low in the front, you could play with ring height until you got the scope level.
I ended up putting a Skinner barrel mounted peep on my 1886 carbine. I've tried scout scopes before on other leverguns and just never liked it. Eventually I guess I will need the magnification

It shouldn't be a difficult thing for a competent gunsmith to do - most likely as others have said - as a section of 'rail' - all that has to be done is to use a stock rail that is on a fairly high riser (which I've seen many of), and mill that riser to compliment the barrel contour. Then the drill/tap stuff that they should be good at.
